    The factor VII activating protease G511E (Marburg) variant and cardiovascular risk

    A previous study had shown a strong relationship between a variant in factor VII activating protease (FSAP G511E) and advanced carotid atheroma. In-vitro, the variant has reduced fibrinolytic but normal pro-coagulant activity, which may constitute a prothrombotic state. The current study has addressed risk for coronary heart disease in a prospective study of cardio vascular disorders (Northwick Park Heart Study II). An interactive effect upon risk was found between the 511E allele and elevated levels of cholesterol and triglyceride. Fibrinogen could substitute for triglyceride levels in this risk-interaction analysis. The findings support the proposal that the FSAP 511E allele exacerbates atherosclerosis or its clinical sequelae

    Exercise-induced respiratory muscle work: Effects on blood flow, fatigue and performance

The official published version can be obtained from the link below.In healthy subjects, heavy intensity endurance exercise places substantial demands on the respiratory muscles as breathing frequency, ventilation and the work of breathing rise over time. In the highly trained subject working at high absolute work rates, the ventilatory demand often causes varying degrees of expiratory flow limitation, sometimes accompanied by lung hyperinflation and, therefore, increased elastic work of breathing. Time-dependant increases in effort perceptions for both dyspnea and limb discomfort accompany these increased ventilatory demands. Similar responses to endurance exercise but at much lower exercise intensities also occur in patients with COPD and CHF. Note that these responses significantly influence exercise performance times in both health and disease. This effect was demonstrated by the marked reductions in the rate of rise of effort perceptions and the enhanced exercise performance times elicited by unloading the respiratory muscles using pressure support ventilation or proportional assist mechanical ventilation. In healthy fit subjects, unloading the inspiratory work of breathing by about one half increased performance by an average of 14% (Harms et al. 2000), and in CHF and COPD patients performance time more than doubled with respiratory muscle unloading (O’Donnell et al. 2001). Why are effort perceptions of limb discomfort markedly reduced and exercise performance increased when the respiratory muscles are unloaded? Our hypothesis is shown in Fig. 1

    Increased plasma markers of oxidative stress are associated with coronary heart disease in males with diabetes mellitus and with 10-year risk in a prospective sample of males

    Background: Increased oxidative stress is associated with coronary heart disease (CHD). We examined the association between plasma markers of oxidative stress and CHD in a cross-sectional sample of patients with diabetes and prospective CHD risk in a sample of men predominantly without diabetes. Methods: Plasma total antioxidant status (TAOS) and the ratio of oxidized LDL (Ox-LDL) to LDL-cholesterol (LDL-C) were determined in a cross-section of 761 Caucasian individuals with diabetes (UDACS study). Plasma TAOS was also determined in 310 baseline samples from a 10-year prospective cohort of 3012 healthy males (NPHSII). Results: Within UDACS, males with CHD had lower mean (SD) plasma TAOS [no CHD, 43.4 (13.2)%; CHD, 40.3 (13.8)%; P = 0.04]. The prevalence of CHD was higher in the lowest compared with the upper quartiles (32.7% vs 19.7%; P = 0.004). We observed a significant association between plasma Ox-LDL:LDL-C and CHD status [no CHD vs CHD, 16.9 (3.1) vs 19.3 (5.0) units/mmol; P = 0.04], with the prevalence of CHD being higher among men in the upper compared with lower quartiles (18.4% vs 35.1%; P = 0.003). No association was observed in females. In NPHSII, TAOS was lower in those who developed CHD [35.1 (8.0)% vs 37.1 (7.9)%; P = 0.04]. The odds ratio for CHD in the lowest compared with the upper quartile was 1.91 (95% confidence interval, 0.99–3.70; P = 0.04). This remained unchanged after adjustment for classic risk factors. Conclusions: A cross-sectional and prospective association exists between baseline plasma measures of oxidative stress and CHD risk. The association with prospective CHD risk remained after adjustment for "traditional" risk factors, implying an independent role for oxidative stress in CHD risk

    Semiotics of alterity and the cultural dimensions of Bible translation

    Translated sacred writings from various religious traditions often retain a few selected cultural terms borrowed from the incipient sign system, while other cultural dimensions are translated in ways that can broadly be construed as domestication. By contrast, many Bible translation agencies eschew translation strategies in which cultural terms are borrowed, advocating in stead for wholesale domestication. In this article, we develop a theoretical framework for representing the alterity, but not the foreignness, of the Bible in translation. Alterity involves the incipient sign system, namely the biblical languages and their cultural contexts ranging from Iron Age Israel within the context of the Ancient Near East for the Old Testament to Roman Palestine in the first century for the New Testament. Examples from African contexts, including Afrikaans (South Africa), Lokaa (Nigeria) and Tira (Sudan), illustrate multiple approaches to representing alterity and provide an important corrective to current practice in many Bible translation projects.

    Measurement of the pubic portion of the levator ani muscle in women with unilateral defects in 3â D models from MR images

    ObjectiveDevelop a method to quantify the crossâ sectional area of the pubic portion of the levator ani muscle, validate the method in women with unilateral muscle defects, and report preliminary findings in those women.MethodMultiâ planar proton density magnetic resonance images of 12 women with a unilateral defect in the pubic portion of their levator ani were selected from a larger study of levator ani muscle anatomy in women with and without genital prolapse. Threeâ dimensional bilateral models of the levator ani were reconstructed (using 3â D Slicer, version 2.1b1) and divided into iliococcygeal and pubic portions. Muscle crossâ sectional areas were calculated at four equally spaced locations perpendicular to a line drawn from the pubic origin to the visceral insertion using the Iâ DEAS® computer modeling software.ResultsThe crossâ sectional area of the muscle on the side with the defect was smaller than the normal side at all the four locations. The average bilateral difference was up to 81% at location 1 (nearest pubic origin). Almost all of the volume difference (13.7%, P = 0.0004) was attributable to a reduction in the pubic portion (24.6%, P < 0.0001), not the iliococcygeal portion (P = 0.64), of the muscle.ConclusionsA method was developed to quantify crossâ sectional area of the pubic portion of the levator ani perpendicular to the intact muscle direction.
